Tesamorelin is a synthetic analogue of human growth hormone-releasing hormone modified with a trans-3-hexenoyl group at the N-terminus to resist DPP-4 degradation. Across two 26-week phase 3 trials in HIV-associated central fat accumulation it significantly reduced waist circumference and visceral adipose tissue — roughly an 18% VAT reduction — without a clinically significant effect on subcutaneous adipose tissue. Later randomised work reported reductions in liver fat. It remains the reference compound in the literature for selective visceral fat research.
